Documentation Needed for Surrendering a Pet

Allow us to provide the best care that we can for your pet by including any information that you think we should know about the animal. The more information we have about the medical history, behavior history, and personality, the more we will be able to do for your pet. Anything you can give us is appreciated.

If you need to surrender your pet to our shelter, we’d appreciate it if you provided the following:

  • A valid photo ID of the owner
  • Any pertinent records pertaining to the pet (veterinary records, microchip information, purchasing receipt, etc.)
  • A completed owner-surrender agreement
  • Brief profile of the pet’s habits and behaviors (forms available at the Shelter or by clicking buttons below).
  • Santa Cruz County Animal Shelter requires that the pet’s legal owner relinquish custody of the animal. If you are bringing in an owned pet that is not yours, we require the animal’s actual owner to sign the animal over to the Shelter.

If your pet has any special food, supplies or toys, please bring those in at the time of surrender.

Santa Cruz County Animal Shelter is an open-admission shelter, meaning that we turn no animal away. There is no surrender fee for Santa Cruz County resident pets.
